Credit Card Transaction Handling Fee

Streamline Health Services uses PayPal to process online credit card transactions. There is an additional 3.2% Credit Card Transaction Handling Fee for online payments. The Credit Card Transaction Handling Fee will be displayed within the Shipping & Handling section on the PayPal sales receipt.

 

  

Steps for Online Payments

STEP 1 - Download appropriate registration form: includes class dates and course fees

 

STEP 2 - Choose a class date

 

STEP 3 - Call (949) 713-7711 or email Streamline Health Services for class availability

   

STEP 4 - Fax Course Participant Waiver to (949) 209–1839

 

STEP 5 - Make Online Payment:

 

  •   Choose correct Course Name from the PayPal drop down menu
  •   Type in Client Name and Class Date, then click Pay Now
  •   Review course fee, from registration form, enter correct dollar amount into PayPal 
  •   Complete PayPal Billing questions

  

Classes Fill Up Quickly! In order to guarantee a spot, Streamline Health Services must receive

course registration forms and course fees.

 

 

Policy and Procedures

  •   Refund Policy:

There will be No Refunds once the course fee has been submitted.

This includes: participants who do not successfully complete a course or course prerequisites, participants who do not attend all scheduled class dates and times, participants who do not pass course land and water skills with proficiency

(according to American Red Cross requirements), or participants who do not pass the final written exam with a score of at least 80 percent. In the event of a catastrophic incident, please contact Amy Alexander directly.

 

  •   Class Transfer Fee:

The class transfer fee for CPR, AED, and First Aid courses is $25.00. The class transfer fee for Lifeguard Training and Water Safety courses is $45.00. Participants must notify Streamline Health Services of a class transfer, at least 72 hours prior to the registered first class date. Participants must receive class transfer confirmation from Amy Alexander.

 

  •   Class Cancellation:

Streamline Health Services reserves the right to cancel any class, due to low enrollment numbers. In the event of class cancellation, registered (paid) participants will be notified, by telephone, at least 48 hours prior to the first class date.
